INDIVIDUAL TOURIST

PURPOSE：
Applicant made the arrangements for the travel or tour in Japan.
If applicant request a multiple-entry visa.
Please attach the request letter.

• All documents must be in A4 size.
  Documents of other size must be copied into size A4 and please submit together with the original..
• Photo size is 45×35mm(2×1.4in).
  Application form and photo size were revised.　Please use revised application form and photo(45×35ｍｍ）.
• Please DO NOT USE FRICTION ERASABLE PEN in visa application form and in all documents.
• Please make sure all documents legally obtained and DO NOT SUBMIT FALSIFIED DOCUMENTS.
• WE FREQUENTLY GET QUESTIONS ON "HOW MUCH" money should be indicated on the bank certificate.
  Although the embassy did not release any guidelines on this or any minimum amount. 　It is best to show your biggest but truthful balance.
  Please call for personal appearance, requirements and others.
•  Please DO NOT PURCHASE any form of travel arrangement prior to the release of your visa application results. Delays and Denials do occur.

【REQUIREMENTS】
All documents must be original unless otherwise stated

1: PHILIPPINE PASSPORT

• Broken passport is not accepted.
  At Least 6 months and have at least 2 blank
• Holder's signature required

2: VISA APPLICATION FORM

• Application Form Size: A-4 size(21.0×29.7cm)
  Application form should be filled out all items correctly.
  If items is not applicable, please fill in [N/A].
  Application form without date of application, signature and with blank item will not be accepted.
  For the date of application, please fill in the date of submission to RELI TOURS AND TRAVEL (or to the embassy in case of direct application).
  Embassy may deny the application that is filled out incorrectly or wrong information.
  Please do not used Friction Erasable Pen in Visa Application Form and in all documents.

3: PHOTO

• Photo size is 45×35mm(2×1.4in) with white background.
  Photo must be taken within 6 months.
  Please write applicant's name and birthday on back side of the photo.
  Photo must be pasted on the application form. -- Please put on glue each corners

4:REQUEST for MULTIPLE-ENTRY VISA LETTER

• (Embassy of Japan format/If requesting multiple entry visa).
  if you are a doctor, lawyer, etc., please provide a copy of your ID.

5:BANK CERTIFICATE -ORIGINAL-

• (Average daily balance 6 months or average amount for past year etc. must be shown)
• If the Average Daily Balance (ADB) in the last six months is not indicated,
• bank statement must be submitted to prove transactions within the last six months.

6:ITR COPY (TAX PAYMENT CERTIFICATE)

Form 2316, must have signature of Employer and Employee

7:CERTIFICATE OF EMPLOYMENT　

(with period of employment, salary, position, contact details of HR)

"For BUSINESS OWNER'S"

8:BANK CERTIFICATE -ORIGINAL-

• (Average daily balance 6 months or average amount for past year etc. must be shown)
• If the Average Daily Balance (ADB) in the last six months is not indicated,
• bank statement must be submitted to prove transactions within the last six months.

9:PROOF OF ACTUAL PAYMENT OF TAX MUST BE SUBMITTED IN ADDITION TO THE BIR TAX PAYMENT CERTIFICATE FORM

(Copy) attach copy of the receipt or electronic application receipt proves tax payment.

10:For single Proprietor
(Copy of DTI) For Corporation (Copy of SEC Certificate front page only and GIS) either should provide Mayor’s Permit from the City Hall.

"For STUDENT"

11:BANK CERTIFICATE -ORIGINAL-

• (Average daily balance 6 months or average amount for past year etc. must be shown)
• If the Average Daily Balance (ADB) in the last six months is not indicated,
• bank statement must be submitted to prove transactions within the last six months.

12:COPY OF SCHOOL ID OR CERTIFICATE OF ENROLLMENT IN SCHOOL MUST BE SUBMITTED.

*If unable to submit no (6), (7), (9), (10), above, a letter of explanation about the reason must be submitted but if "Retired, full-time housewife, unemployed is written in the occupationl field on the first of the application form, it is not necessary to submit it.

【In case that guarantor in /outside of the Philippines will shoulder part/all of travel expense】

13:GUARANTEE LETTER

14:BANK CERTIFICATE -ORIGINAL-

• (Average daily balance 6 months or average amount for past year etc. must be shown)
• If the Average Daily Balance (ADB) in the last six months is not indicated,
• bank statement must be submitted to prove transactions within the last six months.

15:PHOTOCOPY OF TAX PAYMENT CERTIFICATE [see (6) or (9) above]

16:EMPLOYMENT CERTIFICATE [see (7) or (10) above]

17:DAILY SCHEDULE / ITINERARY

18:AUTHORIZATION LETTER FOR PSA

( if applicant is 17 years old below must name, signature, & copy of valid id (with signature) of parents/ if parent's of minor is outside of the philippines scanned is acceptable).

【If there is used Japan Visa on passport. The following (19) and (20) are unnecessary. But required to provide orig visa】

19:BIRTH CERTIFICATE

20:MARRIAGE CONTRACT (IF MARRIED)

(Original issued PSA within one (1) year)

【If Birth/Marriage certificate is unreadable, submit】

21:BIRTH/MARRIGE CERTIFICATE ISSUED BY LOCAL CIVIL REGISTRAR COPY ONLY

【If Birth certificate from PSA "LATE REGISTRATION"】

22:BAPTISMAL CERTIFICATE  

(Both original issued within 3 months and Indicate contact number & address if possible)

23:SCHOOL RECORD FORM 137 (Elementary or High school)

(Both original issued within 3 months and Indicate contact number & address if possible)

【If there is NO RECORD of Birth/Marriage certificate in PSA, submit;】

24:BIRTH/MARRIAGE CERTIFICATE ISSUED BY LOCAL CIVIL REGISTRAR AND NEGATIVE CERTIFICATE ISSUDE BY PSA WITHIN 1 YEAR

25:PROOF OF RELATIONSHIP BETWEEN APPLICANT AND GUARANTOR (e.g. Birth Certificates or Photos)

26:ALIEN CERTIFICATE OF REGISTRATION (ACR) PHOTOCOPY issued by the Philippine government

NOTES:

• Bank certificate outside Philippines; Bank Certificate is generally required & if 6 months average balance is not listed on the bank certificate also required Bank statement. if the applicant can only submit Bank statement, submit explanation letter why cannot submit a balance certificate.
  *However, note that deposit & withdrawal detail by itself is less valid as proof than balance certificate.
• For OFW, certificate of employment/contract, residence ID. A letter of explanation must be submitted if ITR is not available.
• Individuals reapplying their visas's but are unable to independently provide BANK CERTIFICATE/ITR must provide PSA docs to prove affinity relationship.
  (copy is acceptable if there used japan visa on passport)

Reminder's:

• ALL DOCUMENTS MUST BE ORIGINAL UNLESS OTHERWISE STATED.
• EXPLANATION LETTERS IS NOT SUBSTITUTE FOR REQUIREMENTS.
• DO NOT PURCHASE ANY FORM OF TRAVEL ARRANGEMENTS SUCH AS AIRFARE AND ACCOMODATION PRIOR TO THE RELEASE OF YOUR APPLICATION. DELAYS AND DENIALS OCCUR.
• ALL DOCUMENTS MUST BE IN A4 SIZE. A DOCUMENT OF REGAL SIZE (OR ALMOST SAME AS A4 SIZE) IS ACCEPTABLE.
